When calling for an appointment, please be prepared to provide the patient's name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable.

Directions to Waterbury office:

Office is at 2024 East Main Street, a brick building (Wheeler Clinic) at the corner of East Main and Harper's Ferry Road. Parking and the entrance is on Harper's Ferry Road.

From Waterbury on I-84: Exit 25. At the light make a left onto Harper's Ferry Road. Go to the end and the Wheeler Clinic brick building is on the left.

From Hartford on I-84: Exit 24. At stop sign at the end of the exit, take a right onto Harper's Ferry Road. Go to the end. On the left is the Wheeler Clinic brick building.

Directions to Hartford office:

From I-84 (East or West): Exit 44. Office is at 645 Farmington Avenue, corner of Farmington Avenue and Prospect Street in the Wheeler Clinic Counseling Services Offices, Level 1.
